Safe Pregnancy Ending by Laminaria and Uterine Discharge in Patients with Cesarean Section During the Second Trimester of Pregnancy

موضوع: هدف از این مطالعه بررسی رتروسپکتیو احتمال افزایش خطر در ختم حاملگی با لامیناریا و تخلیه رحمی در سه ماهه دوم در بیماران با سابقه اسکار رحمی می باشد. روش مطالعه: از سال ۱۹۷۸ تا ۱۹۹۳، ۱۰۶۴ بیمار با سن حاملگی ۲۲-۱۴ هفته که ۷۰ نفر آنها سابقه اسکار رحمی داشته و حاملگی آنان بوسیله لامیناریا و تخلیه رحمی خاتمه یافت مورد بررسی قرار گرفتند. نتایج: هیچ عارضه مهم جراحی نظیر عوارض بیهوشی و سوراخ شدن رحم و پارگی سرویکس در این بررسی وجود نداشت متوسط زمان عمل برای سقط القایی در گروهی که اسکار رحمی داشتند در مقایسه با گروه بدون اسکار مشابه بوده است. (۴۰/ ۸ / ۰۳۴) (۴۲/ ۷/ ۴۴ + ۴ ) ولی از نظر آماری در موارد سقط فراموش شده تفاوت وجود دارد. این اختلاف زمان عمل را در هر دو گروه بیمارانی که از لامیناریا استفاده کردند (گروه سقط القایی و گروه سقط فراموش شده) توضیح داده اند . اینرسی رحمی همراه با خونریزی در ۲ بیماری با سقط القایی و DIC در ۸ نفر که سقط فراموش شده داشتند اتفاق افتاد اما پارگی رحم مشاهده نگردید. نتیجه گیری: به نظر نمی رسد در بیماران با اسکار سزارین قبلی خطر جراحی ختم دیر رس حاملگی (۱۴ - ۲۲ هفته) با لامیناریا و تکنیک تخلیه رحمی افزایش پیدا کند. پارگی رحم، عارضه ختم حاملگی در سه ماهه دوم می باشد. در مورد عدم وجود خطر ختم حاملگی به کمک تکنیک های ایجاد سقط در خانم هایی که اسکار رحمی دارند، گزارش کمی وجود دارد. اگر چه در باره بی خطر بودن لامیناریا و تکنیک تخلیه رحمی اطلاعات بسیار کمی در دسترس است. اما این روش ختم حاملگی در سه ماهه دوم به عنوان یک روش انتخابی و قابل قبول می باشد.
